Primary Care Providers to Complete Problem-Focused Video Visits for our Medicare Advantage Population

As we face the local and national challenges posed by the Coronavirus outbreak (COVID-19), Affinity Medical Group’s primary focus remains on the well-being of our patients.  Our Medicare Advantage patients are our responsibility to care for throughout the year.

Presently, with the COVID-19 shelter-in-place mandate in the San Francisco Bay Area from the County Health Departments, we still need to be able to provide access to our patients; and Telehealth services are an available option.

Moving forward during this unprecedented time, we will be scheduling a video visit for each of your Medicare Advantage patients. The purpose of the video visit is to provide the patient the opportunity to discuss all medical problems they may have with their Primary Care Physician. During the video visit, it is also a good time to address their chronic conditions and health maintenance due. Once you complete the video visit, please encourage your patients to schedule their Annual Wellness Visit for later in the year. Please see the attached PCP Standard Work document for instructions.
