UHA is pleased to announce that we have a newly established Forms Committee led by Chuck Mesrobian.  The committee has taken on the initiative to evaluate all existing patient forms in an effort to create consistency across our system.  Please download the existing approved forms and replace with those currently being used in your clinic. You can find all Approved Forms on UHA’s Intranet.  As more forms are approved they will be added to the intranet and all providers will be notified via the CMO Corner blog posts.
