If your team has initiated a quality or service improvement project that produced extraordinary results and exceeded set targets, this award may be an ideal venue for showcasing your work.
The award documentation format is designed to reflect our organizations’ endorsement of improvement methodologies that result in improved, sustainable outcomes. This year, we have expanded the program to include staff at ValleyCare and UHA.
If you would like to be considered for a quality or service award, please submit your Letter of Intent form. Once received, you will receive specific instructions and an A3 template.
Completed A3 submissions are due on Wednesday, November 13, 2019, by 4:30pm.
The Malinda S. Mitchell Award for Quality and Service award winners will be announced at the annual SHC employee banquet on January 30, 2020, and presented with a plaque. Click here to see a list of all of the past award recipients.
